A recent 2021 report from the International Journal of Oral and Maxillofacial Implants reviewed data from 6 months of follow-up on 166 patients treated with zygomatic implants showed a 96-100% implant survival rate. I find the reported high implant survival rate is high yet there is no report on implant success rates. However the following complications were also reported: 12% sinusitis (range 4-23%), 11% surgical guide failure (range 3-21%), local infection 10% (range 3-18%), and prosthetic complications 5% (range 0-13%).
I fully respect that the Zygomatic concept is simpler than bone regeneration in theory. Other reports I’ve read show higher complication rates than this paper and more complications over the long haul. I certainly doubt general dentists would put up with this level of complications for any procedure they perform, nor would I.
The types of complications with this implant are also harder to manage than typical surgical complications. Further I note that a sinus osteotomy is often still necessary with Zygomatic fixtures: this seems counter to the Zygomatic philosophy to avoid the maxillary sinus. Consequently I will continue to favor bone regeneration in the severely resorbed maxillary arch rather than use this implant method.
